7 STEPS TO A PERFECT VISION:
STEP 1, Complete all of your personal/employer details in the top section of the form
STEP 2, Visit your nearest Boots opticians or independent optician with the prescription form
STEP 3, Boots opticians have the Bollé Safety frames available for you to view and select
STEP 4, Your optician will complete the technical section of the form
STEP 5, The completed form will then be sent to Bollé Safety by your optician for processing
STEP 6, When your safety glasses are manufactured, they will be sent directly back to your optician
STEP 7, Your optician contacts you to arrange an appointment and advise the glasses are ready for collection and final fitting
